• All contact information you are willing to publicly share
• Southern Illinois counties in which you can perform or teach
• Description of performances or workshops you can provide
• Biographical statement
• Website or facebook links
• A photograph of you/your performing ensemble
• Up to 3 additional candid photographs of you in a teaching or performing environment
• A workshop of performance fee schedule if you have one.
